Important upcoming health deadlines for your student

On April 21, families received an email from Lakeside School via info@lakesideschool.site with a link to complete their student’s health forms. Be sure to check your junk or spam folder if you do not see it in your inbox. You may also access August Schools through the Veracross Family Portal.

Your child’s health and safety are at the center of everything we do at Lakeside. The 2026-2027 Annual Health Packet is now open in Lakeside's online health platform August Schools, and completing it ensures we are fully prepared to support your student from the first day of school.

All student health forms are submitted exclusively through August Schools. Magnus Health was discontinued in September 2025. 

Why it matters

Completing the Annual Health Packet ensures:

  • Strong emergency preparedness
  • Eligibility for athletics, summer programs, and school activities
  • Compliance with state requirements for students with medical conditions

Even if your student’s health has not changed, completing the packet each year keeps us prepared and aligned.

Key deadlines for the 2026-2027 school year

  • June 1, 2026: For families of new students.
  • August 1, 2026: For families of returning students.

Students cannot attend school or participate in activities until the required health documentation and consents are complete.

How to access the Annual Health Packet

  1. Check your email.

    On April 21, families received an email from Lakeside School (info@lakesideschool.site) with a direct link to their student’s health forms. Click “You have a new form request” to log in and begin.
     
  2. Prefer another way to log in? You may also:
  • Log in to the Veracross Parent Portal and select the August Schools tile on the right-hand side (this logs you in automatically using your Veracross credentials), or
  • Visit the Parents and Guardians page on the Lakeside website and open the August Schools tab.

If you're not logged into Veracross, you may be asked to request a one-time login link using your email. Click here for help logging in.

Frequently Asked Questions

What do I need to complete the Annual Health Packet?

Your child’s doctor’s information, insurance details, and their most up-to-date immunization record. If your student is returning, you may not need to update their immunization records. 

What if my child’s allergy or medical condition is not listed?

Select “Other” and use the free text box to enter the details.

If I fill out a medication form online, do I also need to submit a paper copy?

No. Just bring the medication in its original container during drop-off. We will verify the electronic form at that time.

The medication form and athletics physical still say “standing.” Do I need to do anything? 

No. “Standing” means the form remains available in your student’s profile throughout the year in case it becomes needed later.

These forms are only required if your child participates in Upper School athletics or needs medication access at school. If they do not apply to your student at this time, you may leave them as “standing.” You do not need to complete or clear them.

If your student’s situation changes during the year, you can return to August Schools and complete the form at that time.

How do I know it is complete?

You'll know it's complete when you see the following message, reading "All forms complete. Thank you!"

Can I start and stop the form if needed?

Yes, your progress will be saved, and you can return to complete it later.

What if I need language support?

August Schools can be used in other languages. Families can adjust the portal language using the built-in translation tool. See the August Schools language support help article
